Ablative mucosectomy is the procedure of choice to prevent Barrett's cancer.

چکیده
OPENING ARGUMENT Stark strategies face patient and clinician in the presence of a degenerating dysplastic Barrett’s mucosa. Surgical removal offers an overaggressive flawed approach reminiscent of the politics of “Total War” (Karl Von Clausewitz 1780–1831: “Vom Kriege”). Decisive action assures eradication but without definitive knowledge of the threat and incurring extensive collateral damage. Surveillance waits for the cancer to develop; then offers a belated radical solution. Selective mucosal ablation is an active directed therapy offering a targeted pre-emptive preventative strike. I propose that mucosal ablation is currently the procedure of choice to prevent Barrett’s cancer.

متن کاملThe effect of laparoscopic fundoplication in therapy of Barrett's esophagus
INTRODUCTION Barrett's esophagus is the most significant precancer of the esophagus. Its malignization gives rise to most adenocarcinomas of the esophagus. Therefore selection of adequate therapy for this precancerous condition is of the utmost importance. AIM The authors of the work addressed the question of whether effective therapy of reflux disease alone may halt the process of malignizat...

BACKGROUND AND AIMS Advances have occurred in the development of safe and effective ablative therapies for Barrett's esophagus. The aim of the current study was to perform an economic analysis evaluating the cost-effectiveness of endoscopic ablation of nondysplastic Barrett's esophagus. METHODS A Markov model evaluated three competing strategies in a hypothetical 50-year-old cohort with nondy...
متن کاملEndoscopic mucosal resection of Barrett's oesophagus containing dysplasia or intramucosal cancer.
Barrett's oesophagus is premalignant. Oesophagectomy is traditionally regarded as the standard treatment option in the presence of high grade intraepithelial neoplasia or intramucosal cancer. However, oesophagectomy is associated with high rates of mortality and morbidity. Endoscopic ablative therapies are limited by the lack of tissue for histological assessment, and the ablation may be incomp...
متن کاملManagement of Barrett's esophagus.
Barrett's esophagus represents replacement of normal distal esophageal squamous epithelium with specialized columnar epithelium containing goblet cells. Typically arising in the setting of chronic gastroesophageal reflux disease, the presence of Barrett's esophagus carries a 50- to 100-fold increased risk of developing esophageal cancer. Risk factors include male sex, smoking history, obesity, ...
